Air pollution linked to worse rheumatoid arthritis symptoms, study finds
A new study has found that exposure to fine particulate matter, especially PM2.5, may increase disease activity and the risk of painful flares in people with rheumatoid arthritis, suggesting air quality should be considered in clinical care and public health policy.

Air pollution, particularly fine particulate matter known as PM2.5, may make rheumatoid arthritis more active and increase the likelihood of painful flare-ups, according to a new study published in the Annals of the Rheumatic Diseases, a journal of EULAR, published by Elsevier. The findings suggest that air quality could be an important consideration for healthcare professionals treating rheumatoid arthritis and for policymakers working to protect public health.
Understanding rheumatoid arthritis
Rheumatoid arthritis (RA) is a chronic autoimmune condition that causes inflammation and damage in the joints, and can also cause widespread symptoms affecting other parts of the body. The disease affects an estimated 0.5% to 1% of adults worldwide. Its development is driven by a combination of genetics, changes in immune function, and environmental exposures. Smoking is already recognized as a major risk factor, and earlier population studies have associated polluted air with a greater likelihood of developing RA.
The South Korean study
Researchers in South Korea wanted to determine whether air pollution could also affect disease activity and trigger flares in people who already had the condition. The prospective cohort study followed 1,070 people with RA at a major medical center over four years, from 2021 to 2024. During that time, data were collected from 12,583 outpatient visits conducted under real-world clinical conditions. The team estimated each patient's exposure to six common air pollutants: sulfur dioxide, nitrogen dioxide, ozone, carbon monoxide, PM10, and PM2.5. These measurements were compared with disease activity and flare outcomes. The analysis accounted for demographics, serologic status, medication use, socioeconomic conditions, and weather-related variables.
PM2.5 emerges as key factor
Higher PM2.5 concentrations were associated with increased disease activity and flare risk, notably after prolonged exposure to elevated PM2.5 over more than two weeks. PM2.5 particles are extremely small, even smaller than red blood cells, and after being inhaled they can pass from the lungs into the bloodstream and travel to organs throughout the body. Researchers say these particles may stimulate excessive production of reactive oxygen species, which place cells under stress and can damage DNA and activate inflammatory responses, potentially worsening RA.
Lead investigator Eun Bong Lee of Seoul National University said the findings have important implications for public health policy, and recommended that patients avoid prolonged exposure to poor air quality, especially high PM2.5 levels. In an accompanying editorial, Jeffrey A. Sparks of Mass General Brigham and Harvard Medical School called the study one of the largest to use robust methods to link air pollutants with RA disease activity. ARD editor-in-chief Josef Smolen of the Medical University of Vienna noted that while the observations come from a Korean population with a specific genetic background and environment, the study is an excellent starting point for understanding environmental factors in RA and serves as a wake-up call that the environment may be a key contributor to pain and inflammation.
